Our Membership Packages

Membership packages include:

  • Unlimited phone call, text, email, and video chat consultations

  • Direct access to Dr. Lane

  • Extended, unhurried appointments

  • Timely appointments with no waiting

  • School, sports, or camp physical paperwork

  • Coordination of medical care

  • Most basic office tests, such as urinalysis or Strep

  • Many procedures 

Newborns

Birth - 2 months

This includes the newborn visit and 1- and 2-month well-visits, as well as any sick or problem-based visits, and lactation services. All visits provided in the comfort of your own home (within 15 mile radius of office).

Monthly fee: $300

Infants

2 months - 12 months

This includes all well visits and sick or problem-based visits, including lactation assistance and feeding or developmental issues. Visits occur in our beautiful Fort Myers office.

Monthly fee: $225

Little Kids

12 months - 5 years

This includes all well visits and sick or problem-based visits, including feeding or behavior issues. Visits occur in our beautiful Fort Myers office.

Monthly fee: $175

Big Kids

5 years - 18 years

This includes all well visits and sick or problem-based visits, including mental health, female health, and other teen issues. Sports physical paperwork provided upon request. Visits occur in our beautiful Fort Myers office.

Monthly fee: $125

WHAT ABOUT INSURANCE?

As a DPC practice, we do not accept insurance. However, your insurance may reimburse YOU for any visits if covered under your out-of-network benefits. Please contact a representative from your insurance company for more information.

Multiple Children

Birth - 18 years

For children sharing one or both parents and/or living in the same home, we offer a 25% discount on the monthly fee for the second (or more) child. Discount will apply to the less expensive membership fee(s). Family max of $500/month.
