Financial Policy

PAYMENT IS DUE AT THE TIME OF SERVICE

We
accept cash, personal checks, Visa, Mastercard, and Discover

Payment is due regardless of who brings the child in for service.
If you are
reachable by phone, we can take your credit card information
over the phone and send the receipt home with your child's caregiver.
For separated or divorced parents, financial responsibility belongs to the
parent bringing the child in for treatment. We will not bill the other parent;
it is your responsibility to bring what you will owe when you arrive.


Payment for services not covered by your insurance is due at the time
of your visit. This includes copays, coninsurance, and deductibles.

RETURNED CHECKS
Returned checks are subject to a $30 fee.
Full payment for any returned checks will be due at your next visit.


COPAY: A flat fee, required by your insurance company, that you have to
pay at every visit.


COINSURANCE: A fee you pay based on a percentage of the
reimbursement the office will
receive for providing services to you.
[For example, if the insurancepays $100 for a particular type of visit, and you have a 20% coinsurance,
you will be required to pay $20 at the time of service]
   

DEDUCTIBLE: The amount you have to pay towards your healthcare
before the insurance will pay for
anything. A deductible can be $500 or
$5000. It's very important that you know how much your
deductible is and
 if it has been met.

INSURANCE
Our office will submit all insurance claims for you. Please keep us updated with your current
insurance information, including a copy of your insurance card, the claims address, policy and group numbers.

Proof of insurance must be shown at check-in for every visit. Without proof of insurance, you will be charged for the visit in full. For newborns, proof of application will be expected by the 30-day mark for those still not yet added to the insurance. Most commercial insurance companies allow only 30 days to add your newborn to the plan; please do so as soon as possible. By 2 months of age, all babies without proof of insurance will be expected to pay in full for their 2-month well visit and all visits since birth.

Physical or wellness exam $150

Single-problem sick visit $75

Complicated sick visit or ADHD recheck $100


Vaccine administration $15/each

Venous blood draw $25

Capillary blood draw $5

Lead, hemoglobin, glucose $15/each

Flu, RSV, Strep, Mono testing $25/each

Urinalysis (clean catch/bag) $25

SpotVision or audiometry (not with physical exam) $25/each

Albuterol treatment (includes mask & teaching) $25
